  • Identify material variances and coordinate root-cause analysis with Analytics and functional subject-matter experts.
  • Develop and drive corrective-action plans with named owners, timelines, outcomes, and escalation points.
  • Translate value-based contract terms, quality gates, performance guarantees, and obligations into management metrics.
  • Coordinate Clinical, Risk, Quality, Analytics, Network, Finance, and Operations around provider priorities while maintaining one relationship lead.
  • Identify emerging TCOC opportunities and elevate them into the TCOC pipeline.
  • Monitor data requirements, operational commitments, and contract obligations.
  • Escalate material relationship, clinical, financial, or operational issues to the VP.
  • Support renewal, expansion, remediation, or exit recommendations based on demonstrated performance.
  • Represent assigned accounts in the TCOC Portfolio Review and other governance forums.
  • Accept steady-state ownership of partnerships transitioned from Affordability Program Management.

SUCCESS MEASURES

  • Portfolio performance against TCOC and contractual expectations.
  • Timely, high-quality corrective-action closure.
  • Account-plan and QBR discipline.
  • Resolution of material provider issues.
  • Quality and risk performance.
  • Identification of actionable affordability opportunities.
  • Strength of provider relationships and internal coordination.
